As a Case Manager with Community Support Services, you'll work directly with individuals with developmental disabilities, their families and support teams to identify those needs and build a plan for moving forward.
You'll help connect individuals with services and resources, coordinate with providers, advocate for the people you serve and make sure their Person-Centered Service Plans continue to reflect what matters most to them.
