Case Management
Case Management assists individuals eligible for services from the Division of Developmental Disabilities in finding needed medical, social, educational, and other specialized services.
Case Managers are professionals who are trained in the field of mental health and/or closely related fields.
A Case Manager will get to know the individual and assist with developing a person centered plan. This plan will address specific needs and goals that the person has in his or her life.
The Case Manager will work to locate resources and monitor services that will meet the person’s needs and assist the individual in achieving personal goals.
Other services a case manager can provide include:
- advocacy
- linking to leisure and recreational activities
- requesting services from local, state, and federal programs
- employment opportunities
Community Support Services provides Case Management in the following counties:

- Cedar
- Dade
- St. Clair
