RESIDENTIAL SERVICES

   

Community Support Services operates four (4) group homes offering an array of choices for persons with developmental disabilities. Each home provides a warm, caring atmosphere for anywhere from six (6) to eight (8) individuals in a home-like setting. These homes are located throughout Jasper County.

The homes provide 24-hour a day trained staffing. Staffing levels are based on the needs of the individuals residing in each home.

Individualized Supported Living (ISL) is a form of residential habitation that allows individuals the opportunity for community living regardless of the severity of the individual's disabilities or challenges. Individuals may live alone with their family or guardian or share a living arrangement with other individuals with developmental disabilities. The residence is owned or leased by at least one of the residents or the family of one of the residents. Supports are individually designed and coordinated to enable each individual to reside in the community. Services include:

  • Room and Board
  • Case Coordination
  • Qualified Mental Retardation Professional (QMRP) Supervision
  • Community Integration
  • Direct Support Staff up to 24-hours a day, 7 days per week

Residential Habilitation Services are based on the specific needs of the individual. Activities are provided to meet specific outcomes of an individualized Personal Plan. Some of these activities may include but not be limited to assistance of daily living (ADL) in:

  • Bathing
  • Toileting
  • Transfer and ambulation
  • Skin care
  • Grooming
  • Dressing
  • Extension of therapies and exercise
  • Care of adaptive equipment
  • Meal preparation
  • Feeding
  • Incidental household cleaning and laundry

This service also provides instrumental activity of daily living (IADL), which includes:

  • Shopping
  • Banking
  • Budgeting
  • Using Public Transportation
  • Social interaction
  • Recreation and Leisure activities

Assistance with IADL's include:

  • Accompaniment
  • Coaching and minor problem-solving necessary to achieve increased independence
  • Productivity and inclusion in the community

Personal assistance may also include general supervision and protective oversight.
